Much of what we do at CRST is report to you on cutting edge developments in Ophthalmology. We try to give you a balanced look at what new technology, technique or practice is likely or not to be in your future. And that’s where the real challenge begins. How do we know when to adopt an early technology, when to give up on the same technology, when to stop doing what we are comfortable with and when to avoid something new like the plague? We all know how much impact it can be to be the first one on your block to offer the next big thing. We also know the medico-legal danger of the first one to try something or the last one to give up on the old technology.
So how do we pick the winners? What new technology will we trust for our patients and our practices?
FDA studies are great sources of data. Industry spends millions on each one, they take years and the stakes are high. But remember what an FDA study looks at is safety and efficacy, not the chance of real world success. An FDA study is 10 or so docs doing a few hundred or so patients, the real world is hundreds of docs doing thousands of patients. It is not uncommon for a technology to pass the FDA but fail the real world test, look at Intacs for myopia, LTK, etc.
Who is doing it? Steve Brint and I were the first to do LASIK in the US, the reason I was there was Steve Brint, I trusted him. The reason he was there was Lucio Buratto, Steve knew and trusted Lucio. I carefully look at the investigators and developers of a new technology. Talk to them, ask them the hard questions, look at their track records. Some docs have consistently brought superior tools to us. Identify them.
What is the potential of the technique? Some techniques are like computers, they will only get better, some are not. For example, at first the femtosecond laser was worse than a metal keratome but it had such upside potential. Intacs for myopia really were the opposite, pretty much what you saw at first was what you get.
The patient’s perspective, in the end, is key. At first PRK results were not that much different from good RK studies but you knew the patients would go with the “laser” surgery. And no one showed LASIK was better than PRK, the results were the same, but since LASIK had less pain and faster recovery, you knew which one would grow the industry. For lack of a better word, look at the “elegance” of a procedure. Is the surgery simple, beautiful and elegant? Does the eye look untouched at the end? It really matters.
Finally a few more words on the patient. Always let the patient know just where you are in the process. If a patient is your first with a new technique, let them know. If they are your second or third or whatever, let them know. I’ve never had one back out, if anything they are excited to begin the journey with you. And if course they did decline, you would not have wanted to do them.

Eye Surgery
The eye is a fragile organ, requiring extreme care before, during and after a surgical procedure. An expert eye surgeon must identify the need for specific procedure and be responsible for conducting the procedure safely. Many university programs allow patients to specify if they want to be operated upon by the consultant or the resident / fellow.
Anesthesia is essential for any eye surgery. Local anesthesia is most commonly used. Retrobulbar and peribulbar techniques for infiltrating the local area surrounding the eye muscle cone are used to immobilize the extraocular muscles and eliminate pain sensation. Topical anesthesia using lidocaine topical gel is preferred for quick procedures. In topical anesthesia, patient cooperation is a must for a smooth procedure. General anesthesia is recommended for children, traumatic eye injuries, major orbitotomies and for apprehensive patients. Cardiovascular monitoring is preferable in local anesthesia and is mandatory in general anesthesia. Proper sterile precautions are taken to prepare the area for surgery, including use of antiseptics like povidone-iodine. Sterile drapes, gowns and gloves are a must. A plastic sheet with a receptacle helps collect the fluids during phacoemulsification. An eye speculum is inserted to keep the eyes wide open.
